REPORT: COVID-19 outbreaks in health settings almost doubled, death toll up by 333.7% – Data updated to May 5

Each number included in this tracking data represents a human being who is loved and who has experienced the terrible toll of COVID-19: staff who have gone to work despite the risk and the fear; residents and patients who have contracted COVID-19; families and communities who have lost their loved ones; staff who have made the ultimate sacrifice. You are in our hearts. To the media, the Public Health and Ministry staff who have been tracking and compiling this information for public scrutiny, thank you for your work. To all of you, we give our commitment to advocate to the best of our ability and to do everything we can do for the best protection and care for all affected.
Since the week of March 12, the Ontario Health Coalition has been tracking all people who have been infected with COVID-19 in outbreaks in health care settings including hospitals, long-term care, retirement homes, public health units, clinics and congregate care settings, as well as among other health professionals such as paramedics. This has become a gargantuan task but an important one because it enables us to see more fully than is possible from the government data where the spread in health care is happening and whether infection control measures are working to control it. The evidence shows that by every measure the outbreaks in health care settings are not under control and that more effective measures are needed to protect residents, patients and staff. Both the number of outbreaks and the number of people infected in outbreaks has risen dramatically between our last tracking update April 22 (included data up to April 21) and this update up to May 5.
Here are the key findings from the most recent tracking contained in this report of cases in health care settings across Ontario up to the afternoon of May 5:
- The total number of people infected by COVID-19 in health and congregate care settings including staff and patients/residents is 7,894 as of May 5, up from 3,783 in our last report containing data up to April 21. This total has almost doubled in two weeks.
- The percentage of the total number of Ontarians infected with COVID-19 that are health care workers continues to rise. By May 5, out of a total of 18,722 people confirmed to have been infected 3,013 or 16.1 percent were health care workers according to Public Health Ontario. From April 2 to May 5 the total number health care workers infected increased by 2,784 workers, from 229 to 3,013, and the share of the total number of confirmed cases in Ontario made up of health care workers increased from 9.6 percent to 16.1 percent (an increase of 67.7 percent in a month).
- The total number outbreaks in health care and congregate care settings that the Ontario Health Coalition found as of May 5 is 459 (including 107 resolved) in 403 facilities including hospitals, long-term care homes, retirement homes, Public Health Units, clinics and other congregate care settings. As of May 5 Public Health Ontario was reporting 292 outbreaks in public hospitals and long-term care. (Public Health Ontario does not report aggregate numbers for outbreaks in retirement homes, private hospitals and other health and congregate care settings.) By both measures, this is a significant increase. At April 21, the Ontario Health Coalition had tracked 283 facilities with outbreaks and Public Health Ontario was reporting 160 outbreaks in public hospitals and long-term care homes. (See Chart 2.)
- Among previously announced outbreaks across all health care settings, some have been resolved while other existing outbreaks have spread. Despite the positive fact that some outbreaks have resolved, distressingly, the net result remains that there more outbreaks and significantly higher numbers of people infected in these settings. From April 22 – May 5 we found 107 of 459 outbreaks resolved (leaving 352 active), up from 283 on April 21. Among a number of hospitals and long-term care homes with resolved outbreaks we are finding new outbreaks declared.
- The total number of patients/residents that have been infected by COVID-19 in health and congregate care settings has also increased. By May 5 we have tracked a total of 4,951 patients/residents who have contracted COVID-19 in health and congregate care settings. This is an increase of 3,015 (or 155.7 percent) since April 21 when we had tracked 1,936 patients/residents infected in health and congregate care settings. Public Health Ontario’s data also shows a steep increase. As of May 5, they report 2,830 residents/patients infected in hospital and long-term care outbreaks, an increase of 1,104 (or 64 percent) since April 21 when they reported 1,726 patients/residents infected in hospital and long-term care.
- Tragically, the death toll among patients, residents and staff in health and congregate care settings has also increased dramatically. The Ontario Health Coalition is deeply sorry to report that as of May 5, we found a total of 1,878 patients and residents deceased in outbreaks in health and congregate care settings, up from 433 deceased as at April 21. That increase amounts to a heartbreaking 1,445 residents and patients over two weeks or an increase of 333.7 percent.
- We have added Chart 3 tracking the large outbreaks (more than 10 people infected with COVID-19) in health and congregate care settings. As at April 21 we had found 83 large outbreaks. By May 5 we found 138 large outbreaks: 80 in long-term care homes; 29 in retirement homes, 13 in public hospitals, 15 in other congregate care settings.
Note about Methodology: In order to compile this list, we have tracked the daily updates from the 34 Public Health Units in Ontario, including the epidemiological details and outbreak reports where they are publicly available. We have searched each facility name to identify whether it is long-term care or a retirement home and to find details about the cases reported in the aggregate numbers by Public Health Units. We have tracked and compiled media reports of outbreaks with details from across Ontario that contain credible source information (for example, they cited the Public Health Unit, long-term care home administration or hospital administration). Local Public Health Units report in different ways with varying levels of detail. Some do not provide breakdowns of staff/patients/residents. Local Public Health Units usually post outbreak reports for public hospitals, long-term care and retirement homes. The provincial-level information from Public Health Ontario does not include retirement homes or other health care and congregate care/living settings. We have tried to capture everything we can about retirement homes and other health care settings in addition to hospitals (public and private) and long-term care homes using the methodology outlined here. We have not been able to catch up on shelters, group homes and all the various congregate care settings. For that we apologize and will try to capture as much as we can going forward. It is not clear why Public Health Ontario’s numbers for the outbreaks in long-term care and public hospitals are lower than ours. We have repeatedly checked ours for accuracy and we have provided information in the most transparent way possible. In addition, Public Health Units are reporting retirement home outbreaks separately and these numbers should be available to Public Health Ontario but Public Health Ontario does not report the aggregate numbers for retirement homes. Given the number of outbreaks in these private for-profit homes, it is important that the spread in them is also tracked and that they are subject to improved public health measures (and the support needed) to protect residents and staff. Overall, we have tracked every individual case and report in the most credible manner possible and have included sources for each report in the footnotes.
Province-wide Summary from Public Health Ontario:
• May 5, 2020 – Public Health Ontario reported[1] out of a total of 18,722 people confirmed to have been infected with COVID-19, 3,013 were health care workers, or 16.1 percent. There were 6,507 cases with exposure information pending. There were 1,429 deaths among the total population. There were 223 outbreaks reported in long-term care homes and 69 outbreaks in hospitals. In long-term care there were 3,874 confirmed cases. Of these, 2,576 were among residents and 1,293 were among staff, with 653 deaths among residents and one staff. There were 532 cases among hospital outbreaks, 254 among patients and 271 among staff. There were 40 deaths among patients in hospital outbreaks.
• April 22, 2020- Public Health Ontario reported[2] out of a total of 12,245 people confirmed to have been infected with COVID-19, 1,499 were health care workers, or 12.2 percent. There were another 6,845 results pending among the total population. There was a total of 659 deaths among the total population. There were 125 outbreaks in long-term care homes and 35 outbreaks in hospitals. In long-term care there were 2,339 confirmed cases. Of these 1,587 were residents and 748 were staff with 295 deaths among residents, and one staff. There were 242 total cases in hospital outbreaks, 139 were patients, 101 were staff. There were 13 deaths among patients in hospital outbreaks.
• April 16, 2020- Public Health Ontario reported[3] that 1,068 out of 9,525 Ontarians confirmed to have contracted COVID-19 were health care workers, comprising 11.2 per cent. There were 106 long-term care home outbreaks and 30 outbreaks in public hospitals. Of the 1,854 total cases in long-term care there were 1,229 residents and 621 staff infected. Public Health reported that there were 217 deaths, all of them residents except for one family member/volunteer. Of the 194 total cases in hospitals there were 117 patients and 76 staff. There have been 10 deaths in the hospital outbreaks, all of them patients. One environmental services worker also died at the Brampton Civic Hospital. Public Health said that their investigation revealed that it was likely he did not contract COVID-19 in the hospital.[4] On April 15, it was reported that a Personal Support Worker died.[5] She worked at a long-term care home on the eastern edge of Scarborough. Her passing was not recorded in the numbers from Public Health April 16.
• April 7 2020- Provincial health officials stated that 513 out of the 4,726 Ontario patients were health care workers – 10.9 per cent of cases.6
• April 6 2020- Of the 4,347 confirmed cases in Ontario, 451 of them were among health care workers, or 10.4 per cent.7
• April 2 2020- There were 229 lab-confirmed cases in health-care workers. Ministry of Health provided in response to an inquiry from CBC News revealed 9.6 per cent of the province’s 2,392 confirmed COVID-19 cases involve physicians, nurses, paramedics, personal support workers, long-term care home staff and members of other health-care professions. Provincial officials did not provide a breakdown of Ontario’s confirmed cases involving health-care workers by specific roles, transmission source or region.8
• March 28 2020- Ontario chief medical officer Dr. David Williams reported 91 health-care workers in Ontario had tested positive for COVID-19 since the end of January. He said they were unable to confirm how many of those people were exposed or infected on the job at the health-care facilities.[9] It appeared that some homes were not testing all residents who were sick after the first three (or more) test positive.10
Ontario Health Coalition Tracking by Community:
Algoma District
• April 18 2020 – Although not officially declared an outbreak, it was reported that a health care worker at the Sault Area Hospital tested positive. They have since been in self-isolation.11
• May 7 2020 – Algoma Public Health declared an outbreak at Extendicare Maple View LTC in Sault Ste. Marie, after 3 residents test positive.12
Bradford
• April 3 2020- Public Health confirmed that one resident and one team member at long-term care home, Bradford Valley Care Community, tested positive for COVID-19.13
• April 6 2020- The Simcoe Muskoka District Health Unit reported an outbreak; there were 10 cases of COVID-19 at Bradford Valley Care Community, with cases in nine residents and one staff member, according to Charles Gardner, the local health unit’s medical officer of health. Bradford is northwest of Newmarket.14
• April 7 2020- In his daily update, Dr. Gardner announced that total of 14 residents (up from nine) and two staff members (increase of 1) tested positive for COVID-19 at long-term care home Bradford Valley Care Community.15
• April 9 2020- Dr. Gardner announced that the Health Unit confirmed a case in a third staff member at Bradford Valley Care Community.16
• April 11 2020- Dr. Gardner reported the first death Bradford Valley Care Community, a man in his 90s, as well as six new cases in residents (20 in total, including the recently deceased) and another staff member (4 in total).17
• April 13 2020- Dr. Gardner reported that the Bradford Valley Care Community had cases in 23 of its residents (increase of 3) and 5 of its staff (increase of 1). The five team members were recovering in self-isolation at home.18
• April 14 2020- The Public Health Unit reported a second death from Bradford Valley Care Community, a woman in her 80s, whose case was first reported April 7.[19] One resident was hospitalized.20
• April 17 2020 – Cases rose to 37 at Bradford Valley, including 28 residents and 9 staff. A total of four residents have passed away.21
• April 21 2020 – Two more residents at the Bradford Valley Care Community passed away.22
• May 1 2020 – A total of thirty-four residents and eleven staff have contracted the virus at Bradford Valley Care Community. Nine of those residents have died.23
Brantford
• April 3 2020- The Brant County Health Unit has declared COVID-19 outbreaks at two area nursing homes, St. Joseph’s Lifecare in Brantford and Telfer Place in Paris, northwest of Brantford after an employee at each tested positive for the virus. Dr. Elizabeth Urbantke, Brant’s acting medical officer of health, said both of the staff members were self-isolating at home.24
• April 4 2020- Susan Schutta, vice-president of corporate affairs for Revera (owners of Telfer Place), clarified that at that time there were no confirmed cases of the virus in that home or among employees. However, an agency worker, known as a third-party caregiver, who visited the retirement portion of the complex on March 16 tested positive for COVID-19 on March 30. She said the third-party caregiver didn’t visit the long-term care portion of Telfer Place, which is separate from the retirement area.25
• April 12 2020- The outbreaks have been declared resolved for both Telfer Retirement Home and St. Joseph’s Lifecare Centre.26
• April 28 2020- A new outbreak was declared at St Joseph’s Lifecare Centre, which was the facility’s second outbreak, and there were two staff members who tested positive. Telfer Place LTC also had another outbreak with one resident who tested positive.27
• April 29 2020- A new outbreak was declared at Briarwood Gardens Retirement home with 1 staff member testing positive for COVID-19 on April 29 2020.28
Leeds, Grenville and Lanark District Health Unit
Almonte
• March 30 2020- The Leeds, Grenville and Lanark District Health Unit said it was monitoring a novel novel coronavirus outbreak at a long-term care home in Almonte, southwest of Ottawa. Three residents and one staff member at Almonte Country Haven tested positive for COVID-19.29
• March 31 2020- The Leeds, Grenville and Lanark District Health Unit reported that a man living at the Almonte Country Haven Home had died as a result of COVID-19.30
• April 12 2020- Almonte Country Haven Home had 36 residents and staff workers test positive for COVID-19 and 14 deaths. 31
• April 16 2020- Almonte Country Haven Home had 49 residents and staff workers test positive for COVID-19 and 18 deaths. 32
• April 19 2020- Almonte Country Haven Home had 51 cases and 23 deaths in total.33
• April 20 2020- Almonte Country Haven had 51 cases for COVID-19, and 23 residents had passed away in the 83-resident facility.34
• April 27 2020- The total positive cases from Almonte Country Haven increased from 51 to 61, with 17 staff members and 44 residents infected, including another 2 deaths (up from 23 to 25). 35
Carleton Place
• April 2 2020- The Leeds, Grenville and Lanark District Health Unit reported an outbreak at Stoneridge Manor, a long-term care home in Carleton Place (southwest of Ottawa). They said the virus was confirmed in one of its staff members, who was home in isolation.36
• April 12 2020- Two residents of Stoneridge Manor have died of COVID-19. So far, 29 residents and 19 staff members at the 60-bed facility tested positive for the virus.37
• April 27 2020- The total positive cases in residents from Stoneridge Manor increased from 29 to 51 and in staff decreased from 19 to 12. Four more residents had died (total of 6).38
Perth
• March 31 2020- At the Carolina Retirement Residence, a resident and a staff member at the facility tested positive for COVID-19.39
• April 4 2020- At Lanark Lifestyles, a retirement residence in Perth, a staff member tested positive for COVID-19. According to Dr. Neel Chadha, who works at the residence, the staff member was home under self-isolation.40
• April 13 2020- A resident named Shirley McLean, aged 78, of the Carolina Court Retirement Residence in Perth, passed away as a result of COVID-19.41
• April 13 2020- According to a statement provided by Sienna Senior Living, a total of 27 residents and seven team members have tested positive for COVID-19 as of April 13th. 42
Smiths Falls
• April 4 2020- One staff member who works at both the Van Horne and Willowdale Retirement Residences, both in Smiths Falls (south of Carleton Place, and southwest of Ottawa) tested positive. They were self-isolating at home.43
• April 30 2020- An outbreak was declared at Rosebridge Manor retirement home in the Smith Falls area. No details were provided about the size of the outbreak but it was declared resolved on April 30 2020.44
Chatham-Kent
• March 30 2020- Lori Marshall, president and CEO at the Chatham-Kent Health Alliance hospitals, said that 100 staff and 17 physicians were on self-isolation at home.[45] It appears a number of these were people returning from March break, though the information was not clear.
• April 7 2020 – Lori Marshall confirmed in an email in response to a request for clarification from the local Health Coalition that all staff were cleared. All were self-isolating due to travel. There had been no institutional outbreak.
Durham Region
Ajax
● April 9 2020- One staff member of Winborne Park Long-Term Care Home in Ajax tested positive for COVID-19. The staff member was home in isolation.[46] The home consequently declared an outbreak on the first floor with the date of onset recorded as April 6.47
● May 1 2020- The Durham Region Health Department reported that the outbreak at Winborne Park had ended. In total, 1 resident and 1 staff member had tested positive. 48
Ballycliffe Long-Term Care
● March 31 2020- The Durham Region Health Department reported an outbreak at Ballycliffe Long-Term-Care Residence, also known as Chartwell Ballycliffe, a long-term care home in Ajax, with the date of onset being March 26.49
● April 4 2020- On Saturday, the Durham Region Health Department confirmed one case at Ballycliffe Long-Term Care, a 100-bed home owned by Chartwell.50
● April 6 2020- A spokesperson for the home confirmed that one resident has died of COVID-19.51
● April 13 2020- Four deaths (up from 1) and an additional 16 positive cases were reported in Ballycliffe long-term care home in Ajax. Nine of those who tested positive were staff. 52
● April 20 2020- The Durham Region Health Department reported 64 cases at Ballycliffe Long-Term care home, including 12 deceased. It was unclear how many of these cases were staff or residents.53
● April 30 2020- Ballycliffe LTC had 73 residents and 23 staff members test positive with 21 deaths reported.54
● May 4 2020- Ballycliffe LTC reported 5 more deaths (26 total) and 2 more staff infected (25 total). Of the 73 resident cases, 26 had passed away, 26 were resolved, and 2 were hospitalized, with the remaining 19 in isolation.55
● May 7 2020- Ballycliffe had 74 resident cases (increase of 1) including 27 deaths (increase of 1).56
Beaverton
● March 29 2020- An outbreak was declared at Lakeview Manor long-term care home in the Old Mill Unit, with the date of onset reported as March 27.57
● April 7 2020- One employee at Lakeview Manor tested positive for COVID-19, and all other resident tests have come back as negative as of April 7.58
● April 20 2020- The Durham Region Health Department reported that the 1 staff case at Lakeview Manor had been resolved.59
● April 23 2020- The Durham Region Health Department reported that outbreak at Lakeview Manor had ended. 60
Oshawa
● May 2 2020- The Durham Region Health Department reported that Chartwell Wynfield Retirement Residence had an outbreak after 1 resident died. 61
● May 4 2020- An outbreak at Extendicare Oshawa LTC was declared with 1 resident testing positive for COVID-19. An outbreak at Traditions of Durham Retirement Home- Bayshore TCU was also declared, but no details had been provided.62
Hillsdale Terraces Long-Term Care Home
● March 19 2020- The Region of Durham said two women, who were 80 and 92, and two men, who were 68 and 71, at the Hillsdale Terraces Long-Term Care Home in Oshawa tested positive for the virus on Wednesday March 18. All of the residents were being treated at the facility and were in isolation.
● March 23 2020- The 92-year-old woman at Hillsdale Terraces Long-Term Care Home died in hospital, as confirmed by Durham Region Medical Officer of Health, Dr. Robert Kyle.63
● March 26 2020- A total of eight residents of the Hillsdale Terraces seniors’ home in Oshawa we reconfirmed as having COVID-19, according to the Durham Region Health Department.64
● March 30 2020- An additional death was confirmed- a man in his 90s at Hillsdale Terraces Long-term Care Home on Monday March 30. Durham Region Health Department said the man was tested and confirmed positive for COVID-19 after his death.[65] Another 25 residents of the home were showing symptoms of the virus but had not been tested as of Tuesday, given the previous government guidelines, a spokeswoman for the local health unit said twelve staff members were also off work with respiratory symptoms, including three who tested positive for COVID-19, Glendene Collins said.66
● April 5 2020- Durham Region officials said the facility’s death toll was brought up to six residents. There were 27 confirmed cases of the novel coronavirus at Hillsdale Terraces, including the deceased. Six staff members at the facility also tested positive.67
● April 13 2020- Hillsdale Terraces Long-Term Care Home reported 48 cases of COVID-19 at the home, including eleven residents who had died. Ten staff members had also tested positive (increase of four).68
● April 20 2020- The Durham Region Health Department reported cases in 32 residents, including 12 deaths (20 of these cases were recovered) and 12 staff (3 of these were recovered) at Hillsdale Terraces.69
● April 29 2020- The Durham Region Health Department reported cases in 42 residents (increase of 10; 24 of these cases were recovered, 12 died, and 6 were active cases), and 17 staff (increase of 5; 8 of these were recovered and 9 were active cases) at Hillsdale Terraces.70
Lakeridge Health Ajax Pickering
● May 4 2020- Lakeridge Health Ajax Pickering has declared an outbreak in the LHAP VA4E Surgical Unit with 3 staff members and 2 patients testing positive for COVID-19.71
Lakeridge Health Oshawa
● April 1 2020- Lakeridge Health Oshawa hospital was reported to have an outbreak in area C7, as per the Durham Region website. [72] There was one hospital-acquired inpatient case of COVID-19 and a health care worker treating the patient subsequently tested positive for COVID-19 as well, according to an internal document.73
● April 2 2020- A second patient tested positive in the C7 (cardiac care) unit of Lakeridge Health. The unit was temporarily closed for admissions.74
● April 9 2020- Lakeridge Health Oshawa has declared an outbreak of COVID-19 that occurred at the hospital last week was over.75
● April 24 2020- Durham public health has reported that one patient and three staff members have tested positive at Lakeridge Health Oshawa in a second outbreak in the F4 & F5 Units.76
● May 7 2020- Lakeridge Health Oshawa had cases in 4 patients including 1 death and 2 staff members in the second outbreak. 77
Pickering
Bay Ridges
• April 12 2020- An outbreak was declared at Bay Ridges Long-Term Care Home on the first floor, with the date of onset as April 10. 78
• April 15 2020- One administrative staff member at Bay Ridges was confirmed, on April 12, to have tested positive for COVID-19.79
• April 30 2020- The Durham Region Health Department reported that the Bay Ridges outbreak had been declared over.80
Chartwell Pickering City Centre
• April 5 2020- An outbreak was declared at Chartwell Pickering City Centre Retirement Residence on the fourth floor, with the date of onset as March 31.81
• April 30 2020- The Durham Region Health Department reported that Chartwell Pickering City Centre Retirement had cases in 9 residents including 1 death, and 4 staff.82
• May 4 2020- The Durham Region Health Department reported that two more residents at Chartwell Pickering have tested positive for COVID-19, bringing the total up to 11 residents infected.83
Rene Goupil House
• April 20 2020- The Durham Region Health Department declared an outbreak at Rene Goupil House, a Jesuit retirement community in Pickering, reported to have started on April 14.
• April 30 2020- The Durham Region Health Department reported that Rene Goupil House retirement home had cases in 16 residents, and 3 staff members.84
● May 4 2020- The Durham Region Health Department reported that 5 more residents at Rene Goupil House have tested positive for COVID-19, bringing the total up to 21 residents including three deaths. There were also 4 more staff who tested positive, bringing the total up to 7.85
Orchard Villa Long-Term Care
• March 31 2020- Orchard Villa Long-Term Care declared a facility-wide outbreak on March 31, with date of onset March 29.86
• April 20 2020- The Durham Region Health Department reported 180 active cases (it was unclear if these cases were in residents or staff) at Orchard Villa Long-Term Care. 150 cases were in isolation, 2 were in the hospital, and 28 were deceased. 87
● May 4 2020- Orchard Villa Long Term Care had 197 residents and 87 staff members test positive, and 57 resident deaths reported.88
● May 7 2020- Orchard Villa Long Term Care had 60 deaths in residents, with cases in 197 residents and 87 staff members, and another 2 unspecified cases. Thirteen people remain hospitalized and 103 people have recovered.89
Orchard Villa Retirement Home
• April 11 2020- Two residents living in Orchard Villa Retirement Home (same address as the long-term care home, but separate) in Pickering died due to COVID-19.90
• April 13 2020- Orchard Villa Retirement Home declared an outbreak on April 13 in the Victoria Unit, and the Public Health Unit reported that the outbreak onset was April 9.91
• April 15 2020- Orchard Villa Retirement Home suffered two more deaths (total of four).92
• April 30 2020- Orchard Villa Retirement Home had 26 residents and 9 staff members test positive.93
● May 4 2020- Orchard Villa RH had 28 residents and 9 staff members test positive, and 5 deaths reported. This is an increase of 2 residents and 1 death.94
Uxbridge
• May 1 2020- Uxbridge Cottage Hospital reported on May 1 that they had an outbreak starting April 26 in the Med. Surge Unit. One patient and 3 staff members had tested positive. 95
Reachview
● April 15 2020- The Durham Region Health Department reported an outbreak at ReachView Village Long-Term Care Home in Uxbridge. The date of onset was April 11, for the Cedar Grove Unit.96
● April 30 2020- Reachview LTC has had 5 residents, 2 staff members test positive and 1 death reported.97
● May 4 2020- Reachview LTC had an increase of 1 resident and 4 staff members test positive and 1 more death reported, bringing the total up to 6 residents, 6 staff members, and 2 resident deaths.98
Douglas Crossing Retirement Home
● March 16 2020- Douglas Crossing Retirement Home declared an outbreak that was reported to have started March 9, as per the Durham Region website.99
● April 5 2020- The Durham Region Health Department confirmed Sunday that a resident at Douglas Crossing Retirement Home died. 100

● April 15 2020- The Durham Region Health Department announced an outbreak at Fairview Lodge, a long-term care home in Whitby, for its non-resident care support services staff. The outbreak onset was reported to be April 10. 104
● April 16 2020- The Health Department confirmed that one staff member at Fairview Lodge had tested positive, but they did not provide direct resident care. All other staff who interacted with the infected person were going to be tested, whether they showed symptoms or not.105

• April 1 2020- The Grey Bruce Health Unit confirmed that one employee at Maple View Long-Term Care residence tested positive. The home was put into automatic outbreak.123
• April 3 2020- The Grey Bruce Health Unit confirmed that a second employee at Maple View Long-Term Care residence tested positive. She is a female in her 50s and at home self-isolating.124
• April 9 2020- Richard Franzke, a vice-president with Southbridge Health Care LP (who own Maple View Long-Term Care), made the remark in emailed responses to questions about how staff were coping. He said staff and residents have been infected, the first confirmation that residents had been infected as well. He did not say how many.125
• April 12 2020- The Grey Bruce Health Unit said that a third health-care worker at Maple View long-term care facility, a female in her 50s, tested positive and was home self-isolating.126

• March 24 2020- Wellington-Dufferin-Guelph Public Health (WDGPH) said two employees at Guelph General Hospital (GGH) contracted COVID-19. Hospital officials said they were both self-isolating at home.131
• March 26 2020- Four health care workers at Guelph General Hospital (including the two identified on March 24) and 1 patient were infected and more were expected, said Marianne Walker, CEO of GGH. GGH and WDGPH declared an outbreak of COVID-19 on a medical ward (4E) at the hospital.132
• April 3 2020- There were 24 staff members at Guelph General Hospital who tested positive for COVID-19 as of Friday (April 3) morning. Most of the cases were connected to the outbreak on wing 4E that was discovered the previous week. Dr. Nicola Mercer, Medical Officer of Health for WDGPH, said that to her knowledge all 24 were self-isolating and recovering at home.133
• April 9 2020- Guelph General Hospital said an COVID-19 outbreak in one of their units had been declared over. The hospital said the decision was made on Thursday in consultation with Wellington-Dufferin-Guelph Public Health as it had been 14 days since the last hospital-acquired COVID-19 patient was put into isolation.134
Headwaters Health Care, Orangeville
• March 31 2020- Five staff at the hospital at Headwaters Health Care Centre (HHCC) tested positive for COVID-19. They were at home and were self-isolating for 14 days.135
• April 2 2020- Ten staff at Headwaters Health Care Centre tested positive in total, and 23 were in self-isolation. 32 had been self-isolating, but 9 had returned by Thursday April 2. Among the 10 staff with novel coronavirus were seven nurses on the hospital’s F-wing, two nurses in its intensive care unit and one personal support worker on the E-wing, or long-term care floor.136
• April 6 2020- The number of Headwaters hospital employees confirmed to have tested positive for novel coronavirus has risen from ten to thirteen, while 38 were self-isolating at home, as reported by the Headwaters Health Care Centre.137

• April 6 2020- A resident at Shelburne Residence Retirement and Nursing Home tested positive for COVID-19. Richard Franzke, vice-president of communications with Southbridge Health Care Homes, said the resident had been placed in isolation since coming back from the hospital.146
• April 13 2020- Three residents tested positive for COVID-19 and two deaths at Shelburne Residence Retirement and Nursing Home have been reported among their residents. 147
• April 20 2020- Wellington Dufferin Guelph Public Health reported cases in 14 residents and two staff (increase of 10 residents) at Shelburne Residence. 148

Dufferin Oaks Long-Term Care Home
• March 30 2020- An employee at the Dufferin Oaks home tested positive for COVID-19. According to Dufferin County officials, the staff member hasn’t been onsite since displaying symptoms and was isolating at home. None of the residents presented with symptoms as of March 30. 151
• March 31 2020- A second Dufferin Oaks staff member tested positive for coronavirus.152
• April 6 2020- The County of Dufferin confirmed that a fifth individual tested positive for novel coronavirus at Dufferin Oaks long-term care home in Shelburne. This includes two residents and three staff members. 153
• April 14 2020- Three residents and four staff members have tested positive for COVID-19 and thirteen more were awaiting results. 154

• April 1 2020- Wellington Dufferin Guelph Public Health has declared a COVID-19 outbreak at Guelph’s Homewood Health Centre, a private mental health and addictions facility. As of Wednesday afternoon, two staff members and one patient tested positive.157
• April 4 2020- On Saturday April 4, an employee at Wellington Terrace Long-Term Care Home in Aboyne, northwest of Guelph tested positive for COVID-19. The employee is experiencing mild symptoms and is recovering in isolation at home.158
• April 14 2020- The following facilities in the Wellington Dufferin Guelph Public Health area have reported 37 residents and 31 staff members at the following facilities [159]:

Haldimand and Norfolk
Anson Place
• March 29 2020- A resident from Anson Place, retirement and long-term care facility in Hagersville, was the area’s first death due to the novel coronavirus. The Haldimand-Norfolk Health Unit said that the person had died after being admitted to West Haldimand Hospital in Hagersville.165
• March 31 2020- Two more residents have died and 11 tested positive for COVID-19 at Anson Place Care Centre, as confirmed by the home.166
• April 1 2020- Three residents of Anson Place Care Centre have died of COVID-19. The Haldimand-Norfolk Health Unit said that 12 staff members and 11 residents of Anson Place, including the 3 deceased, tested positive for the disease.167
• April 5 2020- A fourth resident of Anson place died. 37 residents at the 101-person care centre and 21 staff tested positive (35 staff tested negative), cases that the health unit said could be traced back to a funeral. The Haldimand-Norfolk Health Unit is investigating whether a fifth death is related to COVID-19. 168
• April 6 2020- The Haldimand-Norfolk Health Unit confirmed that there was a total of 7 deaths in recent days at Anson Place of which 5 were confirmed to be attributable to COVID-19,” said Medical Officer of Health, Dr. Shanker Nesathurai. 39 residents and 22 staff tested positive for the virus.169
• April 14 2020- Anson Place Retirement and Long-Term Care has had 73 residents and 31 staff members test positive for COVID-19 and there had been 19 deaths.170
• April 19 2020- Anson Place reported its twenty-fourth death. Anson Place executive director Lisa Roth said 21 deaths occurred on the long-term care floor and 3 on the retirement home floor. The number of positive cases was at 71 residents with 50 from Anson’s long-term care centre and 21 in the retirement residence as well as 31 employees.171
• April 22 2020- Anson Place reported its 27th death in their long-term care home.172
Caressant Care Courtland
• April 27 2020- Near Tillsonburg, 1 staff member at Caressant Care Courtland LTC has tested positive for the coronavirus.173
Norview Lodge
• April 5 2020- The Haldimand-Norfolk Health Unit confirmed that one non-clinical staff member at Norview Lodge in Simcoe tested positive for COVID-19. The staff member is in self-isolation at home.174

• April 30 2020- Case Manor Care Community LTC has had one more staff member test positive for COVID-19 bringing the total to 7 staff members and 6 residents. 180
Pinecrest Nursing Home
• March 20 2020- Three residents of a long-term care home in the village of Bobcaygeon northeast of Lindsay, two men, ages 89 and 61 and a 94-year-old woman, had tested positive for COVID-19. Late Friday afternoon, Dr. Lynn Noseworthy, medical officer of health for the Haliburton, Kawartha, Pine Ridge District Health Unit, said the residents live at Pinecrest Nursing Home.181
• March 26 2020- Fourteen employees of Pinecrest Nursing Home in Bobcaygeon had tested positive for COVID-19. Two residents died; one on Tuesday March 24, and one on Wednesday March 25. Sixteen more staff and thirty-five more residents were also showing symptoms. Test results for the sixteen staff were pending. It was not clear how many residents or staff may have had COVID-19 because testing was limited only to residents and staff who showed symptoms.182
• March 29 2020- Seven more Pinecrest residents had died over the weekend as of Sunday evening (March 29), according to Dr. Michelle Snarr, the facility’s medical director, and Dr. Steve Oldridge, a local doctor also treating the residents. All were presumed to have died of COVID-19. A total of 17 staff members had tested positive as of Sunday March 29, with more results pending.183
• March 31 2020- Pinecrest Nursing Home confirmed that three more Pinecrest residents had died, as well as a volunteer who visited her husband every day. A total of 24 staff members had tested positive as of March 31, with ten results still pending. 184
• April 1 2020- Two more residents of Pinecrest Nursing Home died, bringing the total to fourteen deaths at that home, plus one family member.185
• April 2 2020- Two more residents died overnight, confirmed by Dr. Snarr.186
• April 3 2020- Four more residents of a long-term care home in Bobcaygeon, Ont. have died of COVID-19, bringing the death toll at the 65-bed facility to 20, plus 1 family member. The new deaths were confirmed on Friday (April 3) morning by Dr. Snarr.187
• April 4 2020- Two more residents died of the novel coronavirus.[188] A third resident died later in the day.189
• April 5 2020- Three residents died on Sunday April 5, as reported on Monday afternoon. The death toll at Pinecrest climbed to 26. In the 65-bed facility, more than a third of the residents had died.190
• April 15 2020- By April 15, a total of 29 residents at Pinecrest Nursing Home had died (almost half of the residents), and a total of 17 staff members (down from 24) were confirmed positive. [191] Twenty-nine of the 33 deaths due to COVID-19 in the Haliburton, Kawartha, Pine Ridge District have occurred in the Pinecrest Nursing Home outbreak.192
• April 28 2020- Pinecrest Nursing Home has now had 33 residents test positive including the previously reported 29 deaths, and 32 staff members (increase of 15).193
Brighton
• May 6 2020- The Haliburton Kawartha Pine Ridge Health Unit stated in a press release that an outbreak was declared at Maplewood Nursing Home in Brighton after a resident tested positive for COVID-19. The resident became symptomatic and was tested on May 1 with test results received on May 4.194
Lindsay
• May 4 2020- The Haliburton Kawartha Pine Ridge Health Unit reported that a COVID-19 outbreak at Caressant Care Lindsay was declared on May 4th after one asymptomatic resident tested positive for COVID-19.195
Halton
• April 6 2020– The following Halton area long-term care and retirement homes have reported outbreaks as follows:[196]
o Burlington: Park Avenue Manor retirement home in Burlington has one staff member who tested positive.
o Georgetown: Mountainview Residence retirement home reported six cases of Covid-19 with one death and three hospitalizations.; Amica Georgetown retirement home has one resident with a confirmed case.
o Oakville: One staff member tested positive for Covid-19 at Oakville’s Post Inn Village long-term care home; Chartwell Waterford Retirement Residence in Oakville is reporting one resident tested positive.

• March 26 2020- A dialysis nurse at St. Joseph’s Healthcare and Hamilton General Hospital tested positive for COVID-19. 212
• April 6 2020- St. Joseph’s Healthcare Hamilton declared a COVID-19 outbreak after three of its health-care workers in the special care nursery tested positive. They said that no babies or parents in the unit were symptomatic.213

• April 20 2020- An outbreak was declared at St. Peter’s Hospital after a patient tested positive for COVID-19 in the 3 West Palliative Care Unit which had been closed to admissions as a result. All patients in the unit were being tested. The 3 East Palliative Care Unit was also closed to admissions.217
• May 6 2020- St. Peter’s Hospital had cases in five patients and five staff test positive. One patient, an 83-year-old man, died.218

Cardinal Retirement Residence, Hamilton
• April 2 2020- Hamilton Public Health declared an outbreak at Cardinal Retirement Home on April 2, and that it moved seven residents from the home into hospitals.220
• April 6 2020- An 80-year-old woman from Cardinal Retirement Residence died in hospital. Roger Channa, owner of the Cardinal, said he only learned of the death at about 3 p.m. Monday April. By the morning of Sunday April 5, there were 13 infected residents — 10 from the transitional care program and three residents from the retirement home part of the facility. He said five staff members had been tested. Two of the tests were negative and they were awaiting results from the other three.221
• April 13 2020- A fourth resident at Cardinal Retirement Residence died and 22 residents and 13 staff members tested positive for COVID-19.222
• April 16 2020- At Cardinal Retirement Residence the number of confirmed cases in residents jumped from 26 to 45. One additional staff member tested positive, bringing the total number of staff members with COVID-19 to 17.223

• April 17 2020- Emmanuel House Hospice on Stinson Street had two new confirmed cases increasing its outbreak to two residents and five staff infected.227

Heritage Green, Stoney Creek
• March 21 2020- Hamilton Public Health has declared a novel coronavirus outbreak at Heritage Green Nursing Home in Stoney Creek on Saturday March 21. The nursing home confirmed two cases: an 80-year-old resident reported on Wednesday March 18, and a 55-year-old resident confirmed on Thursday March 19.229
• March 24 2020- The 80-year-old Heritage Green Nursing Home resident passed away.230

Another 17 residents were ill Kelly Anderson, a spokeswoman for the City of Hamilton, said. Ten staff members at the home were also sick, with one confirmed case of COVID-19. All symptomatic residents and employees will be tested, she said.232
• April 4 2020- A spokesperson for the City of Hamilton confirmed that a third resident, a 100-year-old woman, who lived at Heritage Green nursing home in Stoney Creek died on Friday. She was not hospitalized.233
• April 17 2020- The total confirmed cases at Heritage Green was 15, with in 12 residents and three in staff.234
• April 18 2020- A 92-year-old man at the Heritage Green Nursing Home died of the novel coronavirus Saturday April 18, according to public health. That was the fourth death at that home.235

• April 5 2020- Wentworth Lodge has become the third Hamilton long-term care home with an outbreak of COVID-19. One staff member tested positive for COVID-19 at the Dundas home, the city said Sunday (April 5) morning. Anyone symptomatic at the home will be tested.237
• April 14 2020- Public Health reported that 39 Residents in LTC or Retirement homes tested positive for COVID-19 and there were 7 facilities that have a current outbreak.238

Belleville
● March 31 2020- Between March 31 and April 1, 2020, Hastings Prince Edward Public Health received confirmation that two staff members at Hastings Manor, a long-term care home in Belleville, and one staff member at Hallowell House, a long-term care home in Picton, have been tested positive for COVID-19. These individuals, as well as close contacts, were self-isolating at home.251
● April 2 2020- Hastings Prince Edward Public Health reported on April 2 that Hastings Manor had another outbreak of COVID-19 from February 22 to February 26. 252
● April 15 2020- Hastings Prince Edward Public Health reported five residents and four staff members tested positive for COVID-19 at Hastings Manor. Hastings County Warden Rick Phillips said the staff was home recovering while the residents who tested positive were all living in the same residential area of the home.253
● April 20 2020- Eight residents (increase of 3) and four staff members tested positive for COVID-19 at Hastings Manor. The outbreak at Hallowell house was declared resolved.254
● April 23 2020- New outbreak has been declared at Moira Place nursing home in Tweed with one case who is suspected to be a staff member.255
● April 28 2020- Ten residents (increase of 2) and five staff members (increase of 1) tested positive for COVID-19 at Hastings Manor.256
● April 29 2020- Belmont Long-Term Care Facility in Belleville escaped an outbreak situation. Hastings Prince Edward Public Health reported that there was evidence of COVID-19 identified in the home April 28, but it was determined Monday April 29 that that evidence was insufficient and the outbreak was declared over.257
● May 7 2020- Moira Place nursing home reported that they were no long in outbreak, after the one staff case was determined to be community-acquired. The staff person had not worked in a month as of May 7, and was in isolation at home.258
Belleville General Hospital
● March 31 2020- Quinte Health Care President and CEO Mary Clare Egberts said an undetermined number of health care workers were in contact with a COVID-positive patient at Belleville General Hospital, who had arrived asymptomatic when they arrived at the hospital for a non-COVID-related matter, but developed symptoms soon after being admitted.259
● April 3 2020- Quinte Health Care- 5 inpatient unit at Belleville General Hospital reported an outbreak after two patients were reported as testing positive for COVID-19 on Friday.[260] Thirty front-line health workers were sent home for 14 days to self-isolate after a risk assessment was conducted by Quinte Health Care.261
● April 14 2020- The COVID-19 outbreak at Belleville General Hospital’s Quinte Unit 5 ended on Tuesday April 14 after the virus was first detected on April 1. 262
Kingston, Frontenac, Lennox and Addington
• March 25 2020- Lakelands Family Health Team, based in Northbrook, Ont., in Addington Highlands, was forced to close its doors after a health care worker at their centre was diagnosed with COVID-19. Kingston, Frontenac, Lennox and Addington Public Health said that one other health care clinic in their catchment area had a health care professional diagnosed with COVID-19 and had closed, but they would not be identifying it.263
• March 30 2020- KFL&A Public Health said at least three cases were found in health-care workers, including one at a family health clinic in Northbrook, Ont., and another at Kingston General Hospital. It is unclear where the third health-care worker is employed.264
• March 30 2020- A health care worker at Verona Medical Clinic near Kingston made her diagnosis public. She started noticing symptoms on March 19, and posted her positive diagnosis on Facebook on March 25. 265
• March 30 2020- Two health care workers at Providence Care Hospital tested positive for coronavirus, as announced by the hospital in a press release Monday evening (March 30). All staff and patients who have had close-contact with the infected workers, who will now be quarantining for 14 days.266
• April 1 2020- One staff member at the long-term care facility Providence Manor tested positive for COVID-19, as well as three staff members from Providence Care Hospital.267
• April 2 2020- Two more health care workers at Kingston General Hospital tested positive for the virus, bring the total of infected workers at that hospital to three.268
• April 15 2020- The outbreaks at Providence Manor and Providence Care Hospital ended. “Staff members who tested positive for COVID-19 had recovered and were back at work or would soon be returning,” said Providence Care on Wednesday, regarding both facilities.269

• April 24 2020- The COVID-19 test for a resident of Providence Manor who launched outbreak protocols at the long-term care facility was determined to be a false positive on Friday April 24, according to Providence Care.271
• April 25 2020- According to KFL&A Public Health, one health-care worker tested positive for the novel coronavirus. Public health would not identify where the health-care worker is employed. Providence Care says there were a total of four health-care workers from Providence Care who tested positive for the virus.272
• April 28 2020- Kingston Health Sciences Centre (Kingston General) said on Tuesday April 28: “There have been a total of four health-care workers from Kingston Health Sciences Centre who have tested positive for the virus. The health-care worker announced yesterday by Public Health is not an employee of KHSC.” There were no outbreaks listed on public health’s novel coronavirus dashboard, so the health-care worker does not work at a long-term care facility.16 health care workers total in Kingston region have tested positive; we account for 11 of them.273

• April 14 2020- The Waterloo Region Public Health Unit reported that Cambridge Memorial Hospital had 4 staff that tested positive. 274
• May 1 2020- Cambridge Memorial Hospital confirmed that 6 staff members have tested positive in total. Four have already been cleared to return to work after self-isolating and testing negative for the coronavirus, and four other staff members are currently awaiting their results. They did not declare an outbreak.275
St. Mary’s General Hospital
• March 19 2020- Region of Waterloo Public Health confirmed an employee at St. Mary’s Hospital, a female in her 20s, tested positive for COVID-19 and is self-isolating at home.276
• March 21 2020- More than 50 nurses at St. Mary’s General Hospital in Kitchener were exposed to a patient who tested positive on Thursday March 19, and the six staff who demonstrated symptoms were referred to public health for “fast track testing”.277
• April 18, 2020- St. Mary’s General Hospital declared an outbreak in two of its units. A third-floor unit was closed to admissions after a patient-to-patient transmission was believed to have occurred. Two patients were confirmed as COVID-19 positive (in addition to patients that have been admitted to hospital with COVID-19 not as a result of patient-to-patient transmission). A second outbreak among a cluster of staff confirmed to have tested positive was traced to a common area on the sixth floor. To date 22 staff tested positive for COVID-19 at this hospital. All were self-isolating at home.278
• April 29 2020- Region of Waterloo Public Health reported that 2 outbreaks at St. Mary’s Hospital had ended (the outbreak on the 6th floor unit ended on April 29, and the one on the 3rd floor unit ended on April 30), with a third outbreak ongoing on the 5th floor Medicine Unit that started on April 23.

• April 4 2020- A spokesperson for Forest Heights Revera said the home had four confirmed cases of the virus in residents, as well as one case in staff. They said that all of the affected residents were isolated, while the staff member was at home self-isolating.

• April 6 2020- One of the residents of Highview Residences who tested positive for COVID-19 died over the weekend, a representative of the company told Global News in a statement on Monday April 6. Six residents and an employee have been infected. All of the COVID-19 cases were at Blair Creek House, one of two houses Highview Residences operates at its Kitchener location.295

• March 14 2020- A London woman who works in health care and has no history of travel tested positive for COVID-19. The Middlesex London Health Unit (MLHU) said the London patient is in her 50s and does not work at a hospital. For privacy reasons, officials would not say where she works or what she does.335
• March 20 2020- Public health officials in London announced that a primary health care worker at Strathroy Medical Clinic was Middlesex-London’s second case. She went into isolation.336
•March 20 2020- London-area public health officials announced a nurse in her 20s at London Health Sciences Centre’s University Hospital tested positive for the virus after returning from a trip to Las Vegas. She was in self-isolation. Several of her colleagues were also sent home for isolation as a precaution.337

• May 4 2020- London and Middlesex County confirmed a total of 85 residents (52 LTC and 33 Retirement Homes) and 46 staff members (33 in LTC and 13 in retirement homes) who tested positive. There were 16 deaths to date in long-term care facilities and 6 in retirement homes. The following facilities have updated information about their outbreaks:355

Long Sault
• May 2 2020- An outbreak was declared at Woodland Villa in Long Sault, west of Cornwall. One resident tested positive for COVID-19.356
Napanee
• March 14 2020- A health care professional from Lennox and Addington County General Hospital tested positive and was sent home for isolation. They had returned from Barbados within the previous two weeks, according to Dr. Kieran Moore, medical officer of health for KFL&A Public Health.357
Niagara Region
St. Catharines Hospital
• April 11 2020- An outbreak of COVID-19 was declared at the St. Catharines Hospital after a health care worker tested positive. Niagara Health today said the outbreak was in an inpatient unit and it has since been closed.358
• April 14 2020- Niagara Health has declared a second outbreak at the inpatient Unit 4A of the St. Catharines Hospital. Niagara Health said one of the staff members tested positive for the virus. The outbreak, which was on the unit that is dedicated to caring for COVID-19 patients, was declared because officials determined the case was health care-associated. The staff member was at home and self-isolating.359
• April 22 2020- Niagara public health declared a third outbreak at that hospital in unit 2A of the hospital Tuesday after a patient tested positive for the virus in what is the first Niagara case of hospital-acquired COVID-19 in a patient.360
• May 1 2020- One of the COVID-19 outbreaks at the St. Catharines Hospital ended. Niagara Health’s Infection Prevention and Control team said the outbreak on inpatient Unit 4A is over after 14 days with no evidence of further transmission and no new cases on the unit. The other two outbreaks were ongoing.361
• May 7 2020- A new COVID-19 outbreak at St. Catharines hospital has been declared in the intensive care unit. Niagara Health spokesman Steve Gallagher confirmed a health-care worker tested positive for the virus. He said the hospital system’s investigation determined the transmission occurred in the hospital.362
Long-Term Care and Retirement Homes
Lundy Manor
• March 30 2020- Niagara’s public health department declared an outbreak at Lundy Manor retirement home in Niagara Falls on March 30. The confirmation of COVID-19 came Wednesday April 1.363
• April 3 2020- Three residents of Lundy Manor (retirement home) died, as per Niagara Falls MPP Wayne Gates, who said he was told by the home’s director. On Saturday April 4, Dr. Mustafa Hirji, Niagara’s acting medical officer of health, said there were 13 confirmed cases associated with Lundy Manor.364
• April 20 2020- Lundy Manor had cases in 41 residents including 15 deaths (increase of 12), and 8 staff. 365
Royal Rose Place
• March 31 2020- Niagara’s public health department declared an outbreak at Royal Rose Place LTC in Welland. The confirmation of COVID-19 came Wednesday April 1.366
• April 20 2020- At the Royal Rose Place LTC, 56 residents and 23 staff have been infected to date as per Niagara Health, and 8 residents had died. 367
• April 28 2020- The province of Ontario reported that Royal Rose Place had cases in 40 residents including 11 deaths (increase of 3), and 29 staff (increase of 7). The other 10 resident cases were presumably recovered.368
Other Long-Term Care and Retirement Homes Niagara Region
• March 25 2020- A man in his 80s who was a resident at Ina Grafton Gage Village, a retirement home in St. Catharines, has passed away. He was being treated at Niagara Health – St. Catharines Site.369
• March 27 2020- Niagara’s public health department declared a COVID-19 outbreak in Seasons Retirement Community, a retirement home in Welland, on Friday (March 27) evening after an initial test of two symptomatic residents came back positive for COVID-19. The two residents, one of them a woman in her in 80s, were being treated in the St. Catharines hospital.370
• March 29 2020- Niagara’s public health department declared an outbreak at Rapelje Lodge LTC in Welland. The confirmation of COVID-19 came Wednesday April 1.371
• April 3 2020- An outbreak was declared on the second and third floor of Albright Manor (retirement home) in Lincoln, west of St. Catharines, as per the Niagara Region website.372
• April 7 2020- The Public Health Unit reported an additional outbreak at Shalom Manor (retirement home) in Grimsby. On the outbreak report of the Health Unit website, this outbreak was dated April 3 2020. No additional details were provided.373
• April 9 2020- The outbreak at Rapelje Lodge ended on Tuesday April 9.374
• April 14 2020- The Public Health Unit reported 32 Cases in health care workers and 75 long-term care residents. Royal Rose Place, Lundy Manor and Seasons Welland have experienced 22 deaths total in their three facilities. 375
April 16 2020- An outbreak was declared at Garden City Manor LTC in St. Catharines on or prior to April 16.376
• April 20 2020- Niagara Health reported that there were 384 cases in the Niagara Region to date; 33.33% of those cases were residents in Long-Term Care or Retirement Residences. This is approximately 127 cases. 20.1% of cases in health care workers, meaning approximately 77 health care workers tested positive for COVID-19.377
• April 20 2020- Seasons Retirement Community had cases in 17 residents including 5 deaths and 2 recovered, and 9 staff. 378
• April 21 2020- The outbreak at Shalom Manor ended on or prior to April 21.379
• April 23 2020- Garden City Manor no longer appears on Public Health’s list of ongoing outbreaks.380
• April 28 2020- Niagara Health reported that there were 472 cases in the Niagara Region to date; 32.4% of those cases were residents in Long-Term Care or Retirement Residences. This is approximately 153 cases. 21.2% of cases are in health care workers, meaning approximately 102 health care workers tested positive for COVID-19. There were 42 deaths in this region and 85.7% of these deaths were residents in a long-term care home, meaning there has been 36 deaths in resident homes. 381
• April 28 2020- Niagara Health reported that there were additional outbreaks declared at the following facilities 382:

• May 5 2020 – The Woodlands of Sunset LTC and Bethesda Community Services Inc ended their outbreaks, as they no longer were on the Niagara Health outbreak listing.384
• May 6 2020 – The outbreak at Albright Manor ended on or prior to May 6.385
North Bay
• May 4 2020 – An outbreak has been declared at the Cassellholme Home for the Aged LTC in North Bay. The home announced that a resident was confirmed to have the virus, although remaining asymptomatic. The resident and their roommate were transferred to an isolation wing.386
Ottawa
• April 6 2020- Thirty-nine local health-care workers or first responders were among the 370 confirmed cases of COVID-19 in Ottawa, according to Ottawa Public Health.[387] Two are respiratory therapists at the Civic campus in addition to the respiratory therapist reported positive March 22.388
• March 18 2020- A Health Canada employee at Tunney’s Pasture tested positive for COVID-19, the department confirmed Wednesday March 18. The employee went into self-isolation.389
• April 1 2020- An Ottawa paramedic tested positive for the novel coronavirus.390
CHEO
• March 29 2020- A health care worker at Children’s Hospital of Eastern Ontario (CHEO) tested positive for COVID-19. In a statement on its website, CHEO said the individual worked in an Ambulatory Care clinic, and that he was in self-isolation and doing well.391
Montfort Hospital
• April 6 2020- Ottawa Public Health (OPH) announced a COVID-19 outbreak at Montfort Hospital in its Monday afternoon update. According to the OPH website, the outbreak was in “Ancillary Staff.” The Montfort Hospital does not know how the individuals were infected, but all were in self-isolation.392
• April 7 2020- A second outbreak was reported at Montfort Hospital in area “3C”.393
• April 20 2020- Ottawa Public Health confirmed updates on the following outbreaks at the Montfort Hospital:
o Montfort Hospital – ICU (April 10 to April 20)- had 1 staff case that recovered
o Montfort Hospital – 3C (April 5 to April 17)- had 1 staff case that recovered
o Montfort Hospital – Ancillary Staff (April 4 to April 14)- had 1 staff case that recovered394
The Ottawa Hospital
• March 22 2020- A health care worker at the Ottawa Hospital tested positive for the virus by the Eastern Ontario Health Unit. The man was in his 30s, lived in Prescott-Russell and was in self-isolation.[395] It was later confirmed that he was a respiratory therapist working in the Emergency Room at the General Campus.396
• April 5 2020- The Ottawa Hospital declared a COVID-19 outbreak at the Civic campus inpatient unit in areas A5, B5 and AMA after a patient became infected while in hospital. It was unknown how the patient, became infected, the hospital said in a statement.397
• April 7 2020- According to OPH, another Ottawa hospital outbreak affected area 5 West/BMT of the General campus. On its website, the hospital described 5 West as an inpatient unit that cares for leukemia and bone marrow transplant patients.398
• April 20 2020- Ottawa Public Health confirmed updates on the following outbreaks at the Ottawa Hospital campuses:
o The Ottawa Hospital General Campus -5 West / BMT (April 5)- 4 patients including 1 death, and 3 staff
o The Ottawa Hospital Civic Campus -A Five B Five AMA (April 3 to April 14)- had 1 staff case that recovered399
• April 28 2020- Ottawa Public Health confirmed updates on the following outbreaks at the Ottawa Hospital campuses:

• April 20 2020- Ottawa Public Health confirmed updates on the cases in the University of Ottawa Heart Institute hospital in area H5. The outbreak was declared on April 17, with cases in 2 residents including 1 death.402
• April 28 2020- Ottawa Public Health reported that the University of Ottawa Heart Institute had a case in 1 resident (decrease of 1) including 1 death. 403
Long-Term Care and Retirement Homes Ottawa area
Carlingview Manor
• April 14 2020- There were at least two new cases of COVID-19 in residents at the Carlingview Manor long-term care home. The home, which is owned by Revera, first reported a staff member tested positive for COVID-19 April.404
• April 16 2020- Carlingview Manor has 7 reported cases in residents and 7 in staff. 405
• April 20 2020- Ottawa Public Health reported that Carlingview Manor had cases in 36 residents including 3 deaths, and 7 staff.406
• April 28 2020- Ottawa Public Health reported that Carlingview Manor had cases in 96 residents (increase of 60), including 10 deaths (increase of 7), and 38 staff (increase of 31).407
• May 7 2020- Carlingview Manor had cases in 137 residents including 36 deaths, and 73 staff.408
Cite Parkway Retirement Home
• April 12 2020- Ottawa Public Health reported that Cite Parkway retirement home declared an outbreak.409
• April 20 2020- Ottawa Public Health reported that Cite Parkway retirement home had a case in 1 staff member.410
• April 28 2020- Ottawa Public Health reported that Cite Parkway had 2 cases in staff. 411
Élisabeth Bruyère Residence
• April 18 2020- An outbreak was declared at Élisabeth Bruyère Residence, a long-term care home. The Bruyère health organization said one of its employees, who was in self-isolation at home, had worked shifts at the Saint-Vincent and Élisabeth Bruyère campuses and the Élisabeth Bruyère Residence before testing positive.412
• April 28 2020- Ottawa Public Health reported that Élisabeth Bruyère Residence had cases in 6 residents (new), and 1 staff. 413
Empress Kanata
• April 28 2020- Ottawa Public Health reported that Empress Kanata Retirement Home -Memory Unit had 1 case in a resident. The outbreak was declared on April 27. 414
Extendicare Starwood
• April 20 2020- Ottawa Public Health reported that Extendicare Starwood LTC in Nepean, that declared an outbreak on April 13, had cases in 4 staff. 415
• April 28 2020- Ottawa Public Health reported that Extendicare Starwood had cases in 5 staff members (increase of 1). 416
Garden Terrace, Kanata
• March 31 2020- At Garden Terrace long-term care home in Kanata, one worker tested positive for COVID-19.[417] The home declared an outbreak.418
• April 20 2020- Ottawa Public Health confirmed that Garden Terrace LTC had cases in one staff and one resident (increase of one resident). 419
• April 22 2020- The outbreak at Garden Terrace was declared over. 420
• April 23 2020- Ottawa Public Health reported that there was another outbreak at Garden Terrace. There was a case in one staff. 421
Garry J. Armstrong
• April 20 2020- Ottawa Public Health reported 1 staff case at Garry J. Armstrong LTC, with an outbreak declared on April 20. 422
• April 28 2020- Ottawa Public Health reported 3 staff cases at Garry J. Armstrong.423
Laurier Manor
• April 15 2020- Extendicare announced that two staff members at its Laurier Manor location had tested positive for the virus.424
• April 20 2020- Ottawa Public Health reported that Laurier Manor LTC, that declared an outbreak on April 12, had cases in 38 residents and 5 staff. 425
• April 28 2020- Ottawa Public Health reported that Laurier Manor had cases in 50 residents (increase of 12) including 5 deaths, and 27 staff members (increase of 22).426
Madonna Care Community
• April 5 2020- Ottawa Public Health declared a COVID-19 outbreak Madonna Care Community nursing home on April 5.427
• April 19 2020- Two people have died of COVID-19 at Madonna Care Community nursing home, and 30 residents tested positive.428
• April 20 2020- Ottawa Public Health reported that Madonna Care Community had cases in 31 residents including 2 deaths (previously reported), and 4 staff.429
• April 28 2020- Ottawa Public Health reported that Madonna Care Community had cases in 61 residents (increase of 30) including 15 deaths, and 27 staff members. 430
• May 6 2020- Madonna Care Community had 30 deaths related to COVID-19.431
• May 7 2020- An employee at Madonna Care Community in Orleans, an Ottawa long-term care home, died after contracting COVID-19, as per the home’s operator, Sienna Senior Living.[432] They also had an additional 47 residents and 31 staff who tested positive for COVID-19.433
Maison Accueil- Sagesse
• April 20 2020- Ottawa Public Health reported that Maison Accueil- Sagesse which declared an outbreak on April 17, had cases in 2 residents. 434
• April 28 2020- Ottawa Public Health reported that Maison Accueil- Sagesse Retirement Home (previously reported as a long-term care home) had cases in 1 resident and 1 staff. 435
Manoir Marochel Manor
• April 7 2020- An outbreak affecting the entire nursing facility was declared at Manoir Marochel Manor long-term care home.436
• April 20 2020- Ottawa Public Health reported that Manoir Marochel LTC, who announced an outbreak on April 5, had cases in 8 residents and 3 staff as of April 20. 437
• April 28 2020- Ottawa Public Health reported that Manoir Marochel Manor had cases in 35 residents (increase of 27) including 6 deaths (new), and 10 staff (increase of 7). 438
Montfort Long-Term Care Centre
• April 14 2020- The results confirmed 24 of 32 residents in the Montfort LTC home tested positive for COVID-19. Three staff members also tested positive and were at home, in isolation.439
• April 20 2020- Ottawa Public Health reported that Montfort Long-Term Care Centre, who announced an outbreak on April 8, had cases in 33 residents (increase of 9) including 5 deaths (new) and 6 staff (increase of 3). 440
• April 28 2020- Ottawa Public Health reported that Montfort LTC had cases in 86 residents (increase of 53) including 16 deaths (increase of 11), and 21 staff (increase of 15). 441
• May 1 2020- Revera has confirmed that there have now been 24 deaths reported in Montfort Long-term Care.442
Maplewood Retirement Community
• March 29 2020- There was also a confirmed COVID-19 case among residents at a second retirement residence in Ottawa, this time at the Maplewood Retirement Community.443
• March 31 2020- A worker tested positive for COVID-19 at Maplewood Retirement Community, in addition to the previously reported resident. The home declared an outbreak.444
• April 20 2020- Ottawa Public Health reported that Maplewood Retirement Community had cases in 10 residents and 4 staff.445
• April 28 2020- Ottawa Public Health reported that Maplewood had cases in 11 residents (increase of 1) including 4 deaths (new), and 5 staff (increase of 1). 446
• May 4 2020- Ottawa Public Health reported that Maplewood had one more death reported for a total of five. 447
Park Place Retirement Residence
• March 31 2020- At Park Place Retirement Residence, one health care worker tested positive for COVID-19. The home declared an outbreak.448
• April 10 2020- Park Place no longer has outbreak as of April 10. The 1 staff member had recovered. 449
• April 22 2020- Ottawa Public Health reported that Park Place Retirement Residence had another outbreak.450
• April 28 2020- Ottawa Public Health reported that Park Place Retirement Residence had 1 case in a staff member.451
Peter D. Clark LTC
• April 28 2020- An employee at Peter D. Clark long-term care home in Ottawa has tested positive for COVID-19. In a statement, Community and Social Services Department General Manager Donna Gray said the employee last worked on April 24 and is now in self-isolation at home.452
• May 7 2020- Peter D. Clark had a total of the total to six positive tests for residents and five for staff, a memo from Donna Gray, the city’s general manager of community and social services, said Thursday May 7.453
• May 8 2020- Peter D. Clark had a resident pass away due to complications from COVID-19.454
The Perley and Rideau Veterans’ Health Centre
• April 16 2020- The Perley and Rideau Veterans’ Health Centre (LTC) has received confirmation of a second positive test result for COVID-19. The staff member’s last shift at Perley Rideau was on April 12.455
• April 20 2020- The Perley and Rideau Veterans’ Health Centre announced two more workers had tested positive (four in total). 456
April 28 2020- Ottawa Public Health reported that the Perley and Rideau Veterans’ Health Centre had cases in 12 residents (new) including 4 deaths, and 18 staff (increase of 14). 457
Promenade Retirement Residence
• March 27 2020- Ottawa Public Health said a resident at the Promenade retirement residence in Orleans tested positive for novel coronavirus. The individual and their spouse, who also lived at the Promenade retirement residence, were being treated in hospital.458
• March 30 2020- The resident at the Promenade Retirement Residence who tested positive died. Medical Officer of Health Dr. Vera Etches said that there were five confirmed cases at that retirement home.459
• March 31 2020- A second resident at Promenade Retirement Residence in Orleans has died from COVID-19. They were the spouse of the other resident who had died. The home declared an outbreak.460
• April 7 2020- The test of a third resident of Promenade Retirement Residence in Orleans who died a few days ago came back positive for COVID-19.461
• April 20 2020- Ottawa Public Health reported that Promenade had cases in 11 residents including 4 deaths and 1 staff.462
• April 28 2020- Ottawa Public Health reported that Promenade had cases in 2 more staff (total of 3). They still had cases in 11 residents including 4 deaths.463
• May 4 2020- Ottawa Public Health reported that Promenade had cases in 1 more resident for a total of 12.464
Revera Barrhaven Retirement Residence
• April 20 2020- Ottawa Public Health reported that Revera Barrhaven Retirement Residence, which declared an outbreak on April 17, had 1 case in a staff member.465
St. Louis Residence
• April 21 2020- Ottawa Public Health reported that St. Louis Residence LTC declared an outbreak. 466
• April 28 2020- Ottawa Public Health reported that St. Louis Residence LTC had cases in 12 residents including one death, and 2 staff members. 467
Villa Marconi Long-Term Care Centre
• April 12 2020- A staff member at Villa Marconi Long-Term Care Centre in the west end was diagnosed with COVID-19, the home’s operator confirmed on Sunday. They were waiting for the results of 15 further tests, 14 of which were residents, said Joseph Gulizi, UniversalCare Canada’s president and CEO.468
• April 28 2020- Ottawa Public Health reported that Villa Marconi had cases in 3 staff members (increase of 2).469
• May 6 2020- The outbreak at Villa Marconi was resolved on or prior to May 6.470
Waterford Retirement Community
• April 13 2020- Ottawa Public Health (OPH) said Waterford Retirement Community on Bank Street was the latest local institution to be dealing with an outbreak of COVID-19.471
• April 20 2020- Ottawa Public Health confirmed that Waterford Retirement, who declared an outbreak on April 11, had a positive case in 1 staff.472
Other
• April 1 2020- A fifth outbreak in a facility was announced by Dr. Brent Moloughney, associate medical officer of health, at the Settler’s Ridge OCAPDD (Ottawa-Carleton Association for Persons with Developmental Disabilities). They reported two cases as of April 1.[473] That report was later corrected to be one staff confirmed, no clients.474
• April 9 2020- Ottawa Public Health declared that Settler’s Ridge no longer has outbreak as of April 9 2020.475
• April 28 2020- Ottawa Public Health reported that Association Pour L’Integration Sociale D’Ottawa, a non-profit institution, declared an outbreak on April 24 and has 1 case in a staff member.476
May 3 2020- Centre d’Accueil Champlain Roger Seguin declared an outbreak, after 1 staff member tested positive.477
Peel Region
• March 29 2020- The Region of Peel said that as of Sunday, March 29, there were approximately 20 front-line health-care workers in Peel Region who had tested positive for COVID-19 in an email Monday March 30 to the Brampton Guardian.478
• April 5 2020- Peel Public Health said in a news release that a paramedic and his or her partner were exposed to the novel coronavirus while responding to a motor-vehicle collision. Only one of the pair of paramedics tested positive, though both paramedics were in isolation at home.479
• April 29 2020- A third health care worker died. The unnamed woman was a personal support worker at several facilities and was employed by the Victorian Order of Nurses in Peel Region, the Service Employees International Union said Wednesday April 29.480
Hospitals Peel Region
Trillium Health Partners (Credit Valley & Mississauga Hospitals)
• April 1 2020- An outbreak was declared at Credit Valley Hospital after four patients tested positive for COVID-19. They were in a geriatric rehabilitation unit and have been moved to an area that exclusively treats patients with the new coronavirus, the hospital said.481
• April 14 2020- Peel Public Health reported that the Mississauga Hospital confirmed that less than ten staff members tested positive.482
• April 15 2020- As of Wednesday morning, April 15, Trillium Health Partners (THP) has had 76 positive COVID-19 staff cases across its sites. The units impacted were as follows:
o Unit 1D Rehab at Credit Valley Hospital, declared March 31 (declared over on April 14)
o Unit 4B Medicine at Mississauga Hospital, declared April 9
o Unit 3A Medicine at Mississauga Hospital, declared April 11
o Unit 2D Mental Health at Credit Valley Hospital, declared April 12 483
• April 30 2020- Trillium Health Partners reported that the 2 outbreaks at the Mississauga Hospital had ended (Units 3A Medicine and 4B Medicine), and announced three more outbreaks at Credit Valley Hospital with cases in 7 patients and 8 health care workers:
o 1B Medicine: Two patients and two health care workers on Unit 1B Medicine at Credit Valley Hospital tested positive for COVID-19 between April 9, 2020 and April 13, 2020. An outbreak was declared on April 14, 2020. These cases were presumed to be hospital acquired.
o 2B Medicine: Six health care workers and three patients on Unit 2B Medicine at Credit Valley Hospital have tested positive for COVID-19 between April 10, 2020, and April 18, 2020. An outbreak was declared on April 18, 2020. These cases were acquired from a number of sources including community and hospital transmissions.
o 2C Medicine: Two patients on Unit 2C Medicine at Credit Valley Hospital have tested positive for COVID-19 between April 23, 2020 and April 24, 2020. In consultation with Peel Public Health, and based on our current definition, an outbreak was declared on April 25, 2020. These cases were hospital acquired. [484]
o 2C CARU: One health care worker and three patients have tested positive for COVID-19 between April 27, 2020 and May 2, 2020. An outbreak was declared on May 4, 2020. The cases have been acquired from a number of sources including community and hospital.

Village of Erin Meadows LTC
• March 30 2020– The Region of Peel confirmed that the long-term care home Village of Erin Meadows declared a COVD-19 outbreak on March 30. One staff member tested positive.531
• April 14 2020– At the Village of Erin Meadows LTC, 12 residents and 12 staff members tested positive for COVID-19 (up from 1 staff), and three deaths have been reported.532
• April 21 2020– Schlegel Villages confirmed in their daily update that Erin Meadows LTC had 51 cases in residents (increase of 39) including 10 deaths (increase of 7) and 16 staff (increase of 4). 533

Peterborough
• March 30 2020– Peterborough Public Health announced that one case was suspected to be an occupational exposure for a health-care worker who worked outside the city.543
• May 3 2020– St. John’s Centre retirement home in Peterborough is no longer in outbreak as of May 3rd. All staff members and tenants tested negative for COVID-19 as of April 29th. A part-time staff member had tested positive for COVID-19 earlier in April. That employee finished her shift on April 17th and was instructed to stay home on April 20th when she reported symptoms of COVID-19.544
Empress Gardens
• April 27 2020– A resident at Empress Gardens LTC in Peterborough tested positive for COVID-19. Although asymptomatic, they had been tested because they had spent time in the community outside of the home.545
• May 4 2020– Empress Gardens confirmed that they were longer in outbreak. The resident case was a false-positive.546
St. Joseph’s at Fleming Long-Term Care Home
• March 26 2020– A female resident in her 90s at a St. Joseph’s at Fleming, a Peterborough long-term care home, was diagnosed with COVID-19. Peterborough Public Health also confirmed that a staff member at the long-term care home who had symptoms was referred to community paramedics for in-home testing, and was self-isolating at home. 547
• March 30 2020– Three staff members from St. Joseph’s at Fleming were in isolation.548
• April 1 2020– Test results for the staff at St. Joseph’s at Fleming returned with one positive and the rest negative.549
Riverview Manor Long-Term Care Home
• April 3 2020- A novel coronavirus outbreak was declared at Riverview Manor, a long-term care facility. Dr. Rosana Salvaterra, medical officer of health for Peterborough Public Health, said one resident tested positive and was in self-isolation at the facility. Eight other residents were tested, with three coming back negative and five results pending. Five staff members were tested, with one coming back negative and four pending.550
• April 22 2020– The outbreak at Riverview Manor was declared to be over.551
Plantagenet
• April 7 2020– An outbreak was declared at Milieux de vie a L’Érablière (Valoris group home) on the Eastern Ontario Health Unit website. No details were provided.552
• April 9 2020– Milieux de vie a L’Érablière (Valoris group home) had two residents test positive for COVID-19 and were in isolation.553
•May 4 2020– An outbreak was declared at Pinecrest Retirement Residences in Plantagenet on April 22. Eight residents and three staff members tested positive for COVID-19.[554] There have also been 2 deaths reported at this facility. 555
Renfrew County
• March 25 2020– A resident at a retirement home was declared to be the first case in Renfrew County, west of Ottawa. She was in her 90s, and in isolation in Pembroke Regional Hospital.556
• March 29 2020- Renfrew County and District Health Unit confirmed that the resident from the retirement home had passed away.557
• March 31 2020- A staff member at the Miramichi Lodge long-term care home in Pembroke tested positive for COVID-19. The individual was at home in isolation. The County of Renfrew said the employee stayed home after developing symptoms and had not been at work for over a week. Residents and staff that were in contact with the individual have been notified.558
• April 16 2020- The staff member at Miramichi Lodge who tested positive has recovered. Director of long-term care for the County of Renfrew, Shelley Sheedy, added Miramichi Lodge was still considered to be in an outbreak by the Renfrew County and District Health Unit as they wait for test results to come back.559
• April 28 2020- Miramichi Lodge is no longer in outbreak status.560
Sarnia-Lambton
• April 16 2020– At Bluewater Health hospital in Sarnia, eight staff members tested positive for COVID-19.561
• April 16 2020– One staff member at Lambton Meadowview villa tested positive for COVID-19 and the facility was declared in outbreak.562
• May 3 2020- The following facilities had confirmed cases in Sarnia-Lambton:
o Vision Nursing Home LTC- in Sarnia, 4 residents and 2 staff members

The following facilities had their outbreaks declared resolved by the public health unit:

• May 4 2020- Vision Nursing Home LTC has had one death reported bringing the totals to 4 residents and 2 staff members and one death.564
Landmark Village Retirement Home
• March 27 2020– A resident at Landmark Village in Sarnia was confirmed as a new case. The patient was admitted to Bluewater Health March 20, said Steeves & Rozema CEO John Scotland. The independent living retirement home has since sent three to five staff members who came in contact with the man home to self-isolate and self-monitor for 14 days.565
• March 30 2020– Three of the four Sarnia-area deaths from COVID-19 in recent days were elderly people from the same retirement home. Dr. Sudit Ranade, Lambton County’s medical officer of health, confirmed Monday March 30 that there were at least six additional cases at a local retirement facility. John Scotland, CEO of Steeves and Rozema, confirmed Monday the retirement home was its facility, Landmark Village.566
• March 31 2020– Lambton Public Health confirmed on April 1 that a fourth person connected to the Landmark Village seniors’ home in Sarnia was admitted to Bluewater Health Hospital on March 30, and died on March 31. The test result confirmed that individual was positive for COVID-19.567
• April 5 2020– Lambton Public Health confirmed that Landmark Village in Sarnia had 15 confirmed cases- six staff, nine residents, and four deaths (all residents as previously reported).568
• April 6 2020– Five more residents of Landmark Village were confirmed to be infected with the virus, bringing the total number of cases residents to fourteen.[569] It was said later in the day that ten residents were being treated for COVID-19, and that three of the six staff had been cleared to work again.570
• April 14 2020– The recent outbreak at Landmark Village has 23 confirmed cases, seven staff and 16 residents, six of the ten deaths were linked to the retirement home to date.571
• April 20 2020– Landmark Village has 34 confirmed cases of COVID-19- eight staff and 26 residents tested positive.572

• April 21 2020 – Woods Park Care Centre, a long-term care home in Barrie, declared an outbreak.574

Huntsville
• May 8 2020- Chartwell Muskoka Traditions Retirement Residence declared an outbreak after a female resident in her 90s tested positive. 582
Orillia
• April 1 2020- A staff member at the Spencer House, a long-term care home, was confirmed as a new case by Charles Gardner, the unit’s medical officer of health.583
• April 3 2020- The Simcoe Muskoka District Health Unit reported 18 probable cases in symptomatic staff members, and were investigating the sudden deaths of two residents. As of the evening of Friday April 3, ten of those staff and two other residents had been tested with results pending.584
• April 6 2020- Dr. Charles Gardner, medical officer of health for SMDHU, announced that the two deaths were investigated, and were found to have had “underlying conditions” and were not being considered COVID-19 deaths by the health unit. All but two test results had come back, all with negative results for COVID-19.585
• April 10 2020- The outbreak at Spencer House was declared over, as per the Simcoe Muskoka District Health Unit’s website.586
Waubaushene
• April 28 2020 – An outbreak was declared at Bayview Retirement Home in Waubaushene.587
• May 1 2020 – One employee was confirmed positive at the Bayview Retirement Home.588

St. Thomas/Elgin
• March 27 2020– The Health Unit reported an outbreak at Beattie Manor retirement home in Wardsville. No details provided.590
• April 14 2020- A health care worker at St. Thomas Elgin General Hospital tested positive for COVID-19. 591
• April 22 2020– Beattie Manor Retirement (1 staff member) and Caressant Care Bonnie Place Nursing and Retirement Home (LTC-1 staff member) have active outbreaks.592
• May 6 2020- The outbreak at Caressant Care Bonnie Place was resolved between May 3 [593] and May 6.594
Stratford & Area
• March 28 2020- A woman in her late 40s from St. Marys (southwest of Stratford) who worked in health care was announced as one of three new cases by Public Health.595
• March 30 2020- The Chief Medical Officer for Revera confirmed that a resident tested positive at Hillside Manor long-term care home, near Sebringville in the Stratford area. Dr. Rhonda Collins adds one other resident with symptoms tested negative and they were waiting on the test results for a third resident.596
• April 2 2020- Two people living at the Greenwood Court long-term care home in Stratford tested positive for COVID-19, according to Huron Perth Public Health. Both were in isolation.597
• April 6 2020– Medical officer of health Dr. Miriam Klassen Huron, from Perth Public Health, announced that a total of four residents and one member of staff have tested positive for COVID-19 at the Greenwood Court long-term care home.598
• April 7 2020- The public health unit reported the number of confirmed cases at Greenwood Court long-term care home in Stratford was six residents and two staff.599
• April 11 2020- The public health unit reported that one of the residents at Greenwood Court Long-term care home who had tested positive had passed away.600
• April 19 2020- The public health unit reported that Greenwood Court had six residents and five staff who tested positive for COVID-19. The outbreak at Hillside Manor was declared resolved as of April 14 2020. There were two deaths at this facility to date.601
• April 30 2020- The public health unit reported that Greenwood Court LTC (6 residents, 10 staff members and 4 deaths), Blue Water Rest Home LTC (1 resident) and Huronview LTC (1 staff member) have active outbreaks. Hillside Manor LTC’s outbreak was resolved.602
• May 3 2020- Huron Perth Public Health reported an outbreak at Braemar Retirement Centre in North Huron after one staff member tested positive.603
• May 8 2020- Huron Perth Public Health reported that there are 2 staff cases at Braemar Retirement Centre.604
Sudbury
• April 19 2020- Jo-Anne Palkovits, President and CEO of St. Joseph’s Health Centre confirmed on Sunday April 19 that an outbreak was declared at St. Joseph’s Villa long-term care home in Sudbury, where one staff member tested positive.605
• April 28 2020- Outbreaks have been declared at the following facilities:606
o Extendicare York LTC- 1 Staff member has tested positive
o St. Joseph’s Villa LTC- 1 more resident has tested positive
o Pioneer Manor LTC- 4 Staff members and 3 residents have tested positive
• May 4 2020 – A total of 11 positive cases have been confirmed in Sudbury long-term care facilities, including seven employees and four residents. Of these 11 total cases, two have been reported in St. Joseph’s Villa, seven at Pioneer Manor (increase of 3), one at Extendicare York, and one at St. Gabriel Villa LTC (new).[607] There has been one confirmed death- a resident at St. Joseph’s Villa.608
Thunder Bay
• May 2 2020- The Thunder Bay District Health Unit and St. Joseph’s Care Group confirmed that an outbreak was declared on the fourth floor of Hogarth Riverview Manor nursing home, in Thunder Bay. In the most recent testing, one staff member was found to be positive for COVID-19. No residents or other staff members have been identified as having symptoms.609
Timiskaming District
• April 30 2020 – An outbreak was declared at the Northview Nursing Home in Englehart. The Timiskaming Health Unit announced that two residents were confirmed positive, and were stable and in isolation.610
• April 30 2020 – An outbreak was declared at the Temiskaming Lodge retirement home in Haileybury after one resident and three staff members tested positive.611
Timmins Area
• April 11 2020- A COVID-19 outbreak was declared at Spruce Hill Lodge in South Porcupine, with one case. The resident, a woman in her 90s, was in self-isolation at the residence.612
• April 11 2020- A case in a second resident at Spruce Hill Lodge was confirmed to have COVID-19. He is a man in his 80s, as confirmed by the Porcupine Health Unit.613
• April 11 2020- A staff member who tested positive for COVID-19 was at the Extendicare Long-Term Care Home.614
• April 16 2020- The Porcupine Health Unit confirmed that an employee of a Timmins retirement home, St. Mary’s Gardens, tested positive for COVID-19.615
• April 19 2020– Timmins and District Hospital declared an outbreak after a patient tested positive for COVID-19. The outbreak at Extendicare Timmins was resolved.616
• April 23 2020– In a media release sent Thursday morning, the PHU said the woman in her 30s is a staff member at the hospital, although she is identified as not being part of the outbreak. However, a review is being done to determine if there are any potential linkages. She was tested on Tuesday and is currently at home in self-isolation.617
• Apr 23 2020– In total, there were six cases of the virus connected to TADH. Three are patients who had been admitted and the other three were staff members who were self-isolating at home. 618
• April 28 2020– Medical officer of health Dr. Lianne Catton said a man died; he was in his 80s and was identified in the Timmins and District Hospital outbreak. He tested positive April 22 and was admitted to hospital.619

• April 30 2020 – The cases of the two residents at Spruce Hill Lodge and the staff member at St. Mary’s Gardens were resolved. However, these outbreaks remained in effect, as other individuals at the two seniors’ homes remained under investigation.621

Baycrest
• March 20 2020– Baycrest received confirmation that one of their patients tested positive for COVID-19.624
• May 4 2020- Baycrest reported on their website that the 1 hospital patient had recovered from COVID-19. Four staff (2 hospital workers, 1 food services staff, and 1 third-party contractor from The Terraces) were still infected and at home in isolation.625
The Centre for Addiction and Mental Health (CAMH)
• March 30 2020– Two staff members and two patients at The Centre for Addiction and Mental Health (CAMH), a psychiatric hospital in Toronto, tested positive for coronavirus, as confirmed in a statement sent to Global News Monday March 30.626
• April 5 2020– In total, five patients and twelve staff tested positive for COVID-19 at CAMH. A confirmed COVID-19 outbreak was declared on Unit 3-1 (forensic unit) at the Queen Street site. CAMH’s forensic program delivers mental health care to people who come into contact with the law and have been found not criminally responsible. Suspect COVID-19 outbreaks were declared on Unit 3-5 (forensic unit) and on the Geriatric Inpatient Unit A, also at the Queen Street site. The geriatric unit provides mental health care to seniors.627
• April 10, 2020- 14 staff members have tested positive at the CAMH (Queen site) in Toronto, an increase from 12 staff (April 5).628

The Services Employees International Union Healthcare (SEIU) identified the woman who died as 59-year-old Sharon Roberts who at Downsview Long Term Care in North York for 24 years. The union said that Roberts had tested positive for the virus on April 27 and died on Friday May 1.752

• March 25 2020– Two residents of Seven Oaks long-term care home in Scarborough had died, confirmed by Toronto’s medical officer of health, Dr. Eileen de Villa. Of the 249 residents at Seven Oaks, nine were confirmed to have had the virus and four others were “presumed” cases. There were also four staff confirmed positive for COVID-19.901

• March 22 2020– 13 health care workers in Toronto were confirmed to have had the virus including doctors, nurses, and personal support workers from various health care settings. This number was confirmed by Dr. Michael Finkelstein, Toronto’s acting director of communicable disease control and associate medical officer of health.955
• March 30 2020– Eight employees working at Toronto Public Health headquarters, located downtown, had tested positive for COVID-19. TPH confirmed on Monday March 30 that there was an outbreak, and said it has asked all staff who worked at the location between March 20 to 29 to work from home and self-isolate.956

On Tuesday April 28, medical officer of health Dr. Eileen de Villa announced three staff had tested positive at the Jesse Ketchum Early Learning and Child Care Centre in Yorkville, with more tests pending. A spokesperson for Unifor confirmed the death of a 61-year-old Leonard Rodriquez who was an employee of Access Independent Living Services, which provides services for people with physical support needs. Michigan Health Care Staff living in Windsor
• March 21 2020– The second identified case in the region involved a man in his 20s who works in a health care facility in Michigan and resides in Windsor and Essex County. He developed symptoms on March 15 and was tested on March 17. Officials said he had been self-isolating and was recovering at home with mild symptoms.975
• March 31 2020– The Windsor-Essex County Health Unit announced that to date they had 65 confirmed cases, with approximately one third who were health care workers. All but one work in Michigan across the border. The state of Michigan has close to 5 times the number of cases than Ontario, and the City of Detroit has approximately as many cases as the province of Ontario.[976] Historically, there have been 54 individuals working at Windsor Regional Hospital who work in a Detroit hospital as well, and approximately 13 individuals that live in Michigan and only work at Windsor Regional Hospital.977
• April 7, 2020– The Windsor-Essex Health Unit reported 75 confirmed cases among health care staff. Of those, 25 were listed as nursing home staff. There were not updated details about the other 50 positive cases among health care staff in the region.978 (See March 31 note above.)

• March 30 2020- At least 10 inpatients at Windsor Regional Hospital (WRH) have COVID-19; eight of those patients were still at the hospital while two had been discharged. There were 50 inpatients between the two campuses that were showing signs of the coronavirus, but health officials were still waiting for test results to confirm their status.979

A spokesperson for the long-term care home said they were notified on Friday March 27 by a staff member who reported testing positive for COVID-19. • May 3 2020- The Regional Municipality of York reported that Villa Colombo Vaughan had cases in 43 residents including 11 deaths, and 19 staff.1111
• April 7 2020- Villa Leonardo Gambin Care Community, a long-term care home in Woodbridge, said they had 12 confirmed cases of COVID-19 among residents and that three team members tested positive.1112

Yee Hong Centre– Scarborough-McNicoll
• April 13 2020- An employee of the Yee Hong Centre – Scarborough-McNicoll site tested positive after exposure to an active outbreak in a different healthcare facility.1114
• April 22 2020- COVID-19 was detected in a resident of the Yee Hong Centre – Scarborough-McNicoll site. Toronto Public Health confirmed the resident’s positive test on Tuesday evening, April 21, and declared a COVID-19 outbreak. 1115
• May 6 2020- The Yee Hong Centre – Scarborough-McNicoll site was no longer in outbreak, after Toronto Public Health confirmed a negative test for the resident who tested positive. 1116
• March 30 2020- Yee Hong, a Markham long-term care home, had 11 ill residents, five of whom were tested and were waiting for test results, and 10 ill staff members. One resident had been hospitalized, as announced by Dr. Kurji in a news conference on March 30.1117

• April 7, 2020- York Region Public Health reported 41 confirmed cases with institutional transmission on their status report.[1120] Of these, we counted 29 that were over 70 years old and 11 that were listed as being in their 20s through to in their 60s. Among these confirmed cases were 7 deceased. We have tracked 6 of the deceased. A summary report stated that 40 confirmed cases in the region were health care workers. 1121
